Abstrakt		Although atropine is recommended as the initial pharmacological intervention for symptomatic bradycardias, it is not always effective. The optimal emergency treatment of such bradycardias is transcutaneous cardiac pacing. This mode of treatment has now become possible with the incorporation of a demand mode pacing facility into many of our defibrillators. The presented case is that of a patient with symptomatic bradycardia due to third-degree A-V block. The clinical condition did not improve after application of atropine 0.5 mg i.v, so transcutaneous cardiac pacing was commenced . Due to inadequate respiratory effort the patient was intubated before pacing was commenced. The current required for electrical capture was 8OmA. At that current both carotid artery pulses were palpable. The haemodynamic condition of the patient quickly improved. Despite sedation, the patient required additional intravenous analgesia because of marked restlessness. This case showed that the technique of transcutaneous cardiac pacing is relatively uncomplicated and that this method is extremly useful in the treatment of deleterious bradycardias.
