Abstrakt     The accurate diagnosis of acute myocardial infarction (AMI) by the general practitioner remains a difficult clinical problem and is still manly based on the history and interpretation of the electrocardiogram. Because the clinical presentation can be extremely variable, the AMI is not always recognised at first. The aim was to determine positive predictive value, specifcity and sensitivity ofthe initial diagnosis for acute coronary event (ACE) made by general practitioner for the AMI. Design: Retrospective study. Patients and main results: Between 9878 patients treated in Emergency unit for internal medicine of General Hospital Celje in 1996 there were 1219 ( 12,3%) patients with symptoms suggestive of cardiac pathology, who were send by general practitioner. The females in this group were older then men (64,0+-13,9, 57,4+-13,4 respectively, p<0,001). The positive predictive value (41,3%) and sensitivity (46,0%) for the initial diagnosis AMI for AMI was signifcantly higher then for the diagnosis chest pain (5,6%,11,6% respectively) and angina pectoris (5,7%,16,6% respectively) (p<0,05). There was no AMI in the women younger then 55 years with the initial diagnosis chest pain. 45 patients with AMI were not recognised by general practitioner as ACE (22,9% of all AMI). These patients were older (females: 73,4+-11,2, men 63; p<0,001) and their mortality was higher then between others send as ACE. Conclusious: Predictive value and sensitivity of initial diagnosis of ACE made by general practitioner for AMI is low. The right initial diagnosis has important influence on the disease outcome.
