Abstrakt     Skin biopsies performed for isolation of Borrelia burgdorferi sensu lato from erythema migrans (EM) are crucial in microbiological evaluation of antibiotic treatment. In unspecific skin lesions is the isolation of the pathogen usually the only diagnostic tool. We would like to improve our isolation rate of B. burgdorferi sensu lato from the skin that has been by now from the periphery of EM below 50%. In our present study we wanted to assess the center of EM as the possible site for skin biopsy and cultivation of B. burgdorferi sensu lato. In the literature the center of EM is namely not good defined. We included 53 adult patients with untreated typical solitary EM without clinical signs for dissemination. To compare peripheral and central biopsy sites for the isolation of Borrelia burgdorferi sensu lato, two biopsy specimens - one from the margin and the other from the center of the lesion were taken from each of 53 patiemts. Each specimen was dissected on equal parts and cultured in MKP and BSK II medium simultaneously. We isolated borreliae from 25/53 (47%) EM or from 36/106 (34%) biopsy sites or from 42/212 (20%) culture mediums respectively. Borreliae were isolated more often from the center of EM (20/53 - 38%) than from the periphery (16/53 - 30%). Isolates were more frequent from the center also in each medium separately (in MKP medium 19/15 and in BSK II medium 5/3; nonsignificant differences).
