Abstrakt     In this prospective clinical study the importance of synovial fluid analysis in diagnosis of arthritis has been presented. Physiology and pathophysiology of synovial fluid, the joint aspiration technique - indications and contraindications have been explained. The examination of synovial fluid may be divided into the routine (macroscopic and microscopic) and the special ones (microbiologic, imunologic, biochemic and those for studies) with a particular acent to investigations under the polarizing microscope. 91 samples of synovial fluid in patients with synovitis of big joints with joint effusion more than 20 ml were included in this study. The aspirated synovial fluid has been divided into 4 parts for: 1.) macroscopic examination (colour, transparency, viscosity) and an examination under the polarizing microscope, 2.) cytologic analysis, 3.) biochemical analysis (glucose, uric acid, protein analysis and RF - rheumatoid factor), 4.) microbiologic analysis. At the same time blood samples were taken by vein punction (erythrocyte sedimentation, blood count, glucose, uric acid, protein analysis and RF). All samples of aspirated synovial fluids were divided with the regard to the diagnosis into 5 groups. Comparison of laboratoric analysis results between blood and synovial fluid has been made by statistic methods. Synovial fluid samples were classified into 4 groups with the regard to theire characteristics (noninflammatory, inflammatory, septic, haemorragic). (Abstract truncated at 2000 characters).
