Abstrakt     Manipulation encloses a vast number of techniques from the manual therapy. Manipulation is a common term for all forms of passive movement in order to achieve therapeutic results in musculoskeletal disorders. Article presents a short historical view of manipulation and its role in many therapeutic approaches, especialy in orthopaedic manipulation of the spine, fonded by James Cyriax. In orthopaedic manipulation the clinical pathological basis, on which the manipulation is carried out, differs much from other therapeutic approaches also using the manipulation. The latter are very usefull and popular though the manipulation is seldom conducted with caution, responsability and needed knowledge and training. These are the reasons for official medical associations to regard manipulation as a part of alternative medicine and not being very fond of it. Since the studies prove the manipulation being usefull, the world wide dilema remains. Orthopaedic manipulation uses various techniques of manipulation, conducted strictly to the rule and only in case of intervertebral disc instability, with an exception of some other disorders in cervical spine. Those techniques incloude traction, whereas other approaches elements of oscilation instead. Traction adds positive effects while reducing an intervertebral disc fragment back to the anatomic position. Orthopaedic manipulation is conducted after clinical examination and its elements have to be known to the physiotherapist, for they are the basis for chosing techniques and serve as a controle mechanism between manipulations.
