Abstrakt     The authors present a 26-month old girl with myelomeningocele and Arnold-Chiari malformation diagnosed at birth. She is a first child of healthy, non-related parents without family history of neural tube defects. At birth clinical signs and an EMG revealed lesion at L3 level and at two months of age the neurosurgeon has operated her and a large lipoma was excised. At the same time X ray revealed bilateral hip dysplasia and the diagnosis of neurogenic bladder was confirmed by nephrologist and physiotherapy was started, howeverat 8 months of age the girl has entered a complex rehabilitation programme consisting of therapeutic gymnastics, neurodevelopmental treatment, reflex bladder training and parental counselling and instructing. At 17 months of age she was found psychologically normal as testing of cognitive functions, verbal communication skills and social integration revealed no abnormalities for age. She has much improved also regarding motor functions as she was able to sit, to kneel, to hold "all four" position. Relatively good head control, preserved muscle power of limb girdle and trunk as well as the accessories given to her enables her now for vertical stand. At 26 months of age an orthotic device for ankle and foot and reciprocal orthotic device for standing and walking have enabled her to start walking with support. Her own capacities, good co-operation of her parents and especially early multidisciplinary approach are indices of even further improvement in the future. A follow-up video recordings of her successful rehabilitation will be shown at different ages (at 8,17 and 26 months respectively) and the most suitable complex rehabilitation and team approach will be presented.
