Abstrakt     To determine the diagnostic accuracy, sensitivity and specificity of CTA of intracranial vessels and its advantages and disadvantages compared to DSA as the "gold standard" in patients with acute SAH. In prospective study have been included 52 patients with acute SAH. As soon as hemorrhage has been proven with conventional CT scan, we have immediately performed CTA of intracranial arteries. DSA was performed after CTA examination and so had no influence on interpretation of CTA images. According to data from DSA, as well as from surgical findings, we deter- mined sensitivity, specificity and diagnostic accuracy of CTA of intracranial arteries. We also analyzed cases, where the CTA and DSA results did not match and determined advantages and disadvantages of CTA examination. Diagnostic accuracy of CTA was 95% with sensitivity of 93% and specificity of 98%. In three patients, CTA was false negative; in two patients with negative results, a small aneurysm was found in the region of cavernous sinus, and in one patient at the division of pericallosal and callosomarginal arteries. In one patient with a false positive result, DSA showed infundibular widening of posterior comunicating artery. In seven patients that have been operated on the basis of CTA results, we have successfully demonstrated aneurysms as well as intracranial vessels' anatomy. CTA has an important role in detection and preoperative evaluation of intracranial aneurysms in patients with acute SAH, as it is a fast and minimally invasive method. CTA of intracranial arteries has high diagnostic accuracy, sensitivity and specificity.
