Abstrakt     Acute stroke in posterior fossa has typical clinical presentation in the majority of cases. However, in some cases, especially in in the acute cerebellar stroke, the clinical presentation can be unspecific and it is hard to recognise it. Acute vascular disease of brainstem and cerebellum can be life threatening due to compromise of functoin of vital centers (disturbances of counsciuosness, respiratory and circulatory disturbances due to autonomic dysregulation), as well as secondary complications (hydrocephalus, aspiration, central hyperthermia). The majority of complications evolve rapidly and mortality is highest in the acute period of disease. Clinical picture and complications have to be recognised as soon as possible and appropriate therapeutic measures should be initiated immediately by emergancy services. Some instructive cases are presented in the article. This group of patients confirms that rapid and aggressive treatment in the acute period of disease contribute to relatively good outcome of disease, which was considered prognostically bad.
