Abstrakt     The characteristic symptoms of asthma are episodic breathlessness, wheezing, cough and chest tightness. These symptoms are partially or completely relieved by bronchodilators - reversible airflow obstruction. On an admission of a patient that presents with breathlessness and wheezing for the first time a differential diagnosis with other pathological conditions needs to be established. We must consider: COPD, cardiac asthma, mechanical upper and lower airway obstruction (e.g. for- eign bodies, neoplasm, paresis of both recurrent laryngeal nerves, stenosis or malacia of airways, etc), pulmonary embolism, carcinoid syndrome, some systemic diseases and other much more rare conditions. Three different cases in which the diagnose of asthma was suspected are described. In the first case a diffuse airway narrowing with tracheomalacia due to mucopolysaccharidosis was established. In the second case a tumour in left main bronchus was found and in the third case poisoning with food containing histamin was established.
