Abstrakt     68-year old patient was admitted to the emergency room due to severe lumbar pain. Laboratory results were inconclusive, chest X-ray however revealed wide elongated aorta. Musculosceletal pain was diagnosed and the patient was discharged home. He was advised to perform an ultrasound of the aorta in the following days. Three days later the patient was readmitted to the emergency room due to reccurence of lumbar pain. This time he was admitted to the intensive care unit due to a suspected dissection of aorta. Transesophageal echocardiogram, aortography and spiral computer tomography however showed aneurysm of thoracoabdominal aorta. The patient was treated conservatively. Spiral computer tomography also showed pleural effusion on the right side. Due to persistent lumbar pain we decided to perform thoracenthesis of pleural effusion which was haemorrhagic. Rupture of thoracoabdominal aorta was diagnosed and patient was urgently operated.
