Abstrakt		The author reports on a patient with acute left ventricular heart failure. On initial presentation the auscultatory finding was with exception of tachycardia unremarkable. Transthoracic echcardiography showed severe mitral regurgitation. On transesophageal echocardiography rupture of chordae tendineae was established. She underwent urgent surgery with implantation of a mitral valvular prothesis.
