Abstrakt     The incidence of anomalies of the coronary arteries is low. They occur in 0.3 to 1.3 per cent of individuals, and increase in frequency in patients with congenital heart disease. Prompt and accurate interpretation of the results of coronary angiography is essential for the smooth and efficient work of any cardiac catheterization laboratory. Anomalous origin of the circumflex coronary artery is the most common coronary abnormality: the artery arises from the right coronary sinus and passes behind the aorta and pulmonary trunk. In rare instances is it found to emerge from the posterior (non-coronary ) sinus. The anomalous left anterior descending artery arui right coronary artery usually run in front of the great vessels; in a large proportiat of cases, the right coronary artery passes between the aorta and the pulmonary Wnk. Pathologic studies indicate that the abnormal site of origin and tortuousity of the proximal segment predispose anomalous coronary arteries to early development of atherosclerosis. Some coronary abnormalities are associated with a higher rate of myocardial ischaemia and sudden cardiac death. The incidence of sudden cardiac death and ischaemic syndrome is the highest in individuals with a coronary artery trapped between the aorta and pulmonary trunk or arising from the aorta at a sharp angle. Death commonly occurs during exertion as a result of compression of the anomalous artery. The treatment of choice in symptomatic individuals is myocardial revascularisation. Percutaneous translumina) coronary angioplasty (PTCA) is performed whenever the anatomic situation allows for appropriate placement of the guiding catheter. The author presents four patients with anomalies of the coronary arteries demonstrated by coronary angiograms. Patient 1, who sustained myocardial infarction, presented with a common origin of the coronary arteries from the right coronary sinus. (Abstract truncated at 2000 characters).
