Abstrakt     Background. The main purpose of this study is to assess the objectivity and acceptability of transvaginal sonographic cervical evaluation, its possible usefulness in detecting very short and opening cervices in second trimester of pregnancy. The aims of the study are: 1. to compare the results of transvaginal ultrasound examination with the results of clinical examination of the cervix, 2. to examine the influence of transvaginal ultrasound examination of cervix on the frequency of cervical cerclage and on the outcome of these pregnancies, 3. to determine criteria for cervical incompetence. Methods. One hundred twelve singleton pregnant women at risk for cervical incompetence (cases after cervical cone biopsy were not included) comprised in the study group. They were directed from their office gynecologist to the Department of Gynecology and Obstetrics of General Hospital Slovenj Gradec in year 1997, because of suspect clinical examination findings for cervical incompetence, or/and because of history of premature birth in previous pregnancies. In all cases cervical evaluation was made by transvaginal sonographic measurement of cervical length and of eventual spontaneous funneling of the internal cervical os or its changes after application of transfundal pressure. Evaluation was pertormed at 21 weeks of gestation (18 - 23 weeks for the half cases of study group). The sonographic criteria for cerclage were endocervical length <= 20 mm, or/and cervical intemal os funneling (funnel length X width >= 10 mm X 10 mm). Results. In 50,9% (57/112) cases clinical suspicion for cervical incompetence was established with repeated gynecological examination. Transvaginal sonographic evaluation of the cervix reduced percentage of those cases to 14,3% (16/112); this reduction was statistically signifcant (p < 0,025).
