Abstrakt		Elastase (E) a neutral serine protease is released from activated neutrophil leukocytes. After inhibition with his natural alpha 1 proteinase inhibitor (A 1 PI), a stable E-AlPI complex is formed, which can be detected in human body fluids in different infectious and noninfectious clinical conditions. Therefore E-A1PI measures activation of neutrophil leukocytes-first step in inflammatory response and can be used as an early laboratory parameter in neonatal infections (sepsis, pneumonia and meningitis) in critically ill newborns. In our study we tested hypothesis that E-AlPI is an earlier and more sensitive laboratory parameter in the early diagnosis of severe neonatal infections with common neonatal pathogens and coagulase negative staphylococci (CONS), than C-reactive protein (CRP), leukocytes neutrophil series of leukocytes, and clinical signs and symptoms. Also we tested hypothesis that the concentration of E-AlPI is related to number and/or activation (expressed as E-AlPI/neutr L) of neutrophil leukocytes. For that purpose we measured concentrations of E-A 1 PI in plasma on admission ( 1st sample) and 3-5 days later (2nd sample) of 58 severely ill outborn newborns, with the commercially available kit-PMN Elastase IMAC TM (Diagnostica Forschung, Merck, Darmstadt). The patients were allocated (according to Center for Diseases Control from Atlanta- CDC) into 4 groups: I-newborns with proven infection (newborns with isolated microorganisms); II-newborns with suspected infections; III-newborns with suspected infections with severe hypoxia or/and intracranial hemorrhage; IV-control group (newborns with respiratory distress syndrom without isolated microorganisms).
