Abstrakt     Antineutrophil cytoplasmic antibodies (ANCA) are widely used as a useful diagnostic marker for small vessel vasculitides, although the test may be occasionaly positive in various other conditions. The objective of this study was to evaluate ANCA in various clinico-pathologic settings and to analyse renal patohistological changes in relation to ANCA antigen specificity, ANCA were tested by indirect immunofluorescence and enzyme linked immunosorbent assay and were found to be positive in 439 patients in the period from 1989-1990. Renal biopsy samples of 103 patients with an ANCA with microscopic, polyangiitis and pauci-immune necroziting glomerulonephritis was confirmed. Low titres of ANCA, specific for myeloperoxidase and other or unknown ANCA antigens with mostly atypical immunofluorescnece patterns were characteristic for misclelaneous diseases. The pathomorphogenesis of proteinase 3 and myeloperoxidase ANCA associated renal disease seem to be similar. However, more active lesions in the biopsy of patients witn proteinase 3 ANCA and more chronic in those with myeloperoxidase ANCA indicate that proteinase 3 positive renal disease is more severe and clinically more progressive with consecutive earlier decission for the biopsy.
