Abstrakt | Our study was performed on the group of 55 patients with performed stress radioinuclide ventriculography (RNV) with resuIts typical for exertitional ischemia and performed coronarography. At coronarography in 37 pts significant stenosis of one or more coronary arteries was present, in 18 pts there was no signs of significant stenosis. Comparing the results of both subgroups, the authors assessed the sensitivity and specificity for coronary arteries disease for each individual RNV parameter (LVEF at rest, LVEF during stress, dLVEF, EDV at rest, dEDV, regional wall motion abnormality, regional phase delay), for some cikloergometryc (achieved stress level, achieved % of maximal heart rate, stress symptoms), spirometryc (VC and FEV1 reduced >20%, FEV1/VC index) and chest x ray image parameters (hear wide, C/T index, heart volume, pulmonary vascular congestion). All these investigations were performed in the same diagnostical assessment. Aim of the study. 1. Assesment the relative sensitivity and specificity for CAD and the relative importance of each individual parameter of stress RNV and all other listed investigations for RNV result interpretation. Results. I. In RNV diagnostics, the following priority in parameters considering CAD diagnostic values is best sutited: stress achived hipokinesie (sensitivity 67%,specificy 67 %), dLVEF < 5 % (54%,72%), th ecombination of both former parameters. Other RNV parameters follow according to their sensitivity and specificity values: stress phase delay, decreased LVEF at rest, increased EDV at rest, stress EDV at stress. Stress parameters: achieved heart rate less than 75% predicted show high sensitivity and presence of stress symptoms high specificity. (Abstract truncated at 2000 characters.) | |
