Abstrakt     Chronical periapical periodontitis (CPP) is chronic inflammatory periapical lesion in human jaws. It represents the potential inflammatory focus for many illness. The purpose of our study was to investigate the immunological characteristics of CPP in the relations to patohistological and microbiological findigs of CPP more specifically. The hypothesis was that there the hypersensitivity reactions (e.g. hypersensitivity reaction type IV) are taking place in the CPP tissue and they depend on the type of causative bacteria. We obtained after therapeutical surgical removal - apicoectomy 47 tissue samples. All CPP samples were examined by standard histological analysis (HE), the bacteriological analysis of aerobs and anaerobs was made and the lymphocyte subpopulations and their functional status were evaluated by the flow cytometrical (FC) analysis. Sterile were 12,8% bacteriological samples, others had typical aerobic and anaerobic bactreia. With the respect to the most abundant bacterial growth (over 80% of all colonies on the growth plate) we defined two groups of CPP: with the predominance of the obligate anerobic bacteria (anerobic group) and the oral streptococcs (streptococcal group). We used pathohistologic and microbiologic findings and compared them to the results of imunological analysis. The FC of CPP samples showed a significant increase in proportions of T lymphocytes expressing interleukin-2 receptors and ICAM-1 when compared to peripehral blood lymphocytes. A decreased expression in proportions of cytotoxic T lymphocytes (CD 8) expressing interleukin-2 receptors and HLA DR molecules were found in CPP samples predominantly colonized by streptococci, compared with lesions predominantly colonized by strict anaerobes.
