Abstrakt     Objective: The aim of this research was to generate knowledge and hypothesis about the risks related to heroin injecting in the city. The objective of this study was to explore the perception of risk and risk taking behaviour, injection and sexual practices, of Ljubljana IDUs and their possible experiences with treatment. Also, heroin scenes, networks and environmental risk factors were studied. We wanted to find out why HIV seroprevalence among heroin injectors in the city and the country is low and remains low in spite of the previously recorded risk behaviours among injection drug users. Design: The study was qualitative with a grounded theory approach. The elements of the snow-ball sampling and unstructured in-depth interviews were used. Sample: The sample consists of 21 injection drug users (IDUs), both heroin dependent ( n=19) and non-dependent users (n=3) who were selected from the 95 drug users contacted on the IDUs congregation spots of Ljubljana from January 1998 to November 1998. Results: 75 (78.1%) out of 95 persons who completed self-reported questionnaire were involved in risky injection behaviour ( 11 had never injected ). Also, when analysing the data on 21 qualitative interviews more than 17 (81%) out of 21 persons reported that they had shared the equipment at least once during the last few months or were involved in other risky behaviours related to sexual behaviour. The traditional ritual use of one syringe for the group of IDUs is disappearing and is more exception than the rule. The "backloading" and "froantloading" practices were not known or were very rare among the IDUs. Most common risk injection practices were related to the use of same spoon, filter and water. The prevalence of unsafe sexual behaviours in terms of rare condom use was high. (Abstract truncated at 2000 characters.)
