Abstrakt     On account of changes in the age structure of the population as well as modern methods of treatment, heart failure will become the problem of the second half of the 21st century. To confront it, suitable organization and expert knowledge will be required. It will be necessary to follow the indicators of morbidity, mortality and therapeutic success. The paper describes the pathophysiologic elements, with special stress on the particularities of elderly patients. Treatment is based on general nonpharmacologic measures, education and four basic groups of drugs: angiotensin-converting enzyme inhibitors (ACE), diuretics, beta-receptor inhibitors and digitalis in systolic heart failure. Also recommended are aldosterone antagonists and angiotensin II receptor antagonists. Mechanical supportive measures such as pumps and surgical procedures including heart transplantation are not mentioned here. Despite the fact that treatment is basically combined, efforts should be made towards prescribing as few drugs as possible for better patient cooperation and less side effects. Some drugs require significantly lower doses and are adapted to the physiologic requirements of geriatric patients. Most recommendations are based on experiences gained from great studies interwined with our own.
