Abstrakt     Background: The sponraneous course of labour and delivery has been ever more often interfered with. The World Health Organisation and some authors recommended the use of partogram with the ratio between the abscissa (time in hours) and ordinate (cervical dilatation in cm) of 1:1, because decisions taken with the steeper graph are supposed to be less aggressive. Whether decisions on the management of labour taken by the obstetrician and the midwife differ considering the shape of the partogram, is not known. Methods: A total of 2352 women with a singleton pregnancy at 37 weeks gestation age and over were randomly allocated to the partogram with ratio of 4:1 or 1:1. The observed outcome of labour and delivery (duration of labour, operative delivery, birth channel injuries, 1-min Apgar score) was statistically evaluated using the multiple linear regression and logistic regression methods. Results: The analysis showed no effect of the shape of the partogram on the duration of labour (p=0,257), on the operative delivery rate (p=0,760), on the birth channel injuries (p=0,376) and the 1-min Apgar score (p=0,858). Conclusions: On the basis of the results obtained we may conclude that the shape of the partogram does not affect decisions taken by the obstetrician or the midwife on the management of labour.
