Abstrakt     The article describes the thrombolytic treatment of acute ischemic stroke, which is the only possible option available for successful acute treatment of this condition. The general practician or the family doctor plays a major role during the initial phase of the treatment. The time frame from the beginning of symptoms to beginning of therapy is extremely short, i.e. only three hours. During this period, the physician in primary care must recognize acute stroke as an emergency situation, contact the neurologist on duty immediately, and arrange transportation of the patient ro the central clinic. Described is a questionnaire, which can be used in a prehospital setting for identification of patients who qualify under strict conditions for thrombolytic treatment. The results of various clinical studies in the world on thrombolytic therapy and their emerging dilemmas are briefly explained. In the second part of the article, the clinical characteristics of over 100 patients are presented. These patients were admitted to the emergency medical service between 1 October 1999 to 1 April 2000, and were sent to neurologist due to stroke symptoms. 15 patients were sent for thrombolytic therapy. Treatment with altaplaza (t-PA) as a very successful method is shown as an example. It is therefore a must that all patients as well as general public are well informed about the symptoms of stroke equally as heart attack. This would encourage the patients to call at their family doctor or at EMS immediately should they come under such condition.
