Abstrakt     Although the presence of arteriovenous communications in patients with chronic venous insufficiency has been confirmed by several studies in the past, their role in the pathogenesis of this condition is still unclear. On one hand, it is possible that they play a key role in its etiology, but there is also sufficient evidence that their development is secondary to chronic venous insufficiency. The aims of this study were: to prove the existence of arteriovenous communications in 34 patients with chronic venous insufficiency and to establish whether the microembolisation procedure in the muscular branches of the crural arteries can improve the healing of lower venous ulcers in the leg which were resistant to conservative and surgical treatment in 16 cases. This study has provided evidence that arteriovenous shunting is usual in patients with varicose veins and ulcers, and its existence can be demonstrated by digital substraction angiography (31/34 = 91%). Embolisation of the muscular arterial branches which feed the arteriovenous shunts with the use of micro-spirals and microparticles seems to be an effective method for the treatment of nonhealing venous ulcers in lower leg sections (13/16 = 81%).
Izvleček     Prisotnost arterio-venskih povezav pri bolnikih s kronično vensko insuficienco je znana že dalj časa, vendar je njihova vloga v patogenezi še vedno nepojasnjena. Lahko da arterio-venske povezave predstavljajo primarni etiološki dejavnik v nastanku kronične venske insuficience ali pa nastanejo kot njena posledica. Cilj raziskave je bil: s pomočjo digitalne subtrakcijske angiografije ugotoviti prisotnost arterio-venskih povezav pri 34 bolnikih s kronično vensko insuficienco, odkriti, ali mikroembolizacija mišičnih vej golenskih arterij z mikrospiralnimi in mikropartikli pri 16 bolnikih ugodno vpliva na celjenje venskih razjed odzivnih na konzervativno in kirurško zdravljenje. Rezultati raziskave so potrdili prisotnost arterio-venskih povezav pri bolnikih s kronično vensko insuficienco v večini primerov (31/34 = 91%). Mikroembolizacija mišičnih vej golenskih arterij z mikrospiralnimi in mikropartikli je omogočila celjenje večine trdovratnih venskih razjed neodzivnih na konzervativno in kirurško zdravljenje (13/16 = 81%).
