Abstrakt     It is estimated that one in every 75 persons born in 2000 will develop cutaneous malignant melanoma, one of the most deadly forms of human cancer. It is the most often form of cancer in white people aged between 25 and 29. Cathepsin D is an aspartatic proteinase present in the lysosomes of almost all tissues. In malignant cells it is also excreted on to the cell surface. Cathepsin D is believed to be associated with proteolytic processes leading to invasion and seeding of tumor cells. Association between the cathepsin D tissue concentration and the aggressiveness of the tumor have been detected in different cancer types as well as in metastatic cutaneous melanoma. Our aims were to examine the expression of cathepsin D in the tissue of early cutaneous melanoma and in sera of these patients. To estimate tissue and sera concentrations of cathepsin D in early cutaneous melanoma, the concentration of cathepsin D was measured immunoradiometrically (ELSA-CATH-D kit, Cis bio international) in sera and in cytosols of 51 patients with clinically evident primary cutaneous melanomas, as well in as in sera of healthy volunteers and in normal skin. A significantly elevated concentration of cathepsin D was detected in tumor cytosols as compared to adjacent normal tissue cytosols (44.186 versus 14.686 pmol/mgp, p < 0.001). Statistically significant was also the difference of sera cathepsin D concentrations between primary cutaneous melanoma patients (4.074 pmol/ml) and healthy persons (3.508 pmol/ml). Our results indicates that cathepsin D is expressed at high levels by melanoma cells. The extremely high expression of cathepsin D detected in two of our patients with later progression of the disease in a 42 month observation time, suggest the possible correlation between the cathepsin D tissue concentration and the prognosis of primary cutaneous malignant melanoma, thus it might be included among prognostic markers.
