Abstrakt     Background. Ankle fracture is an injury with direct or indirect intraarticular impact. Therefore, the treatment of displaced ankle fractures is as a rule operative. Numerous and heterogeneous factors have influence on the late result of operative treatment of ankle fractures, but prognostic significance of single factors is different. Patients, methods, results. Between 1.1. 1989 and 1.4. 1994 there were 331 displaced ankle fractures operativelv treated in Jesenice General Hospital. All fractures were classified according to AO criteria. In the majority of fractures minimal osteosynthesis was performed. Early ankle exercises without weight bearing were allowed after operation. 242 patients were followed up three to eight years after operation. The late results were from functional, clinical and radiological point of view in 87 % classified as exellent or good. High energy trauma ( p < 0.05 ) and soft tissues complications ( p < 0,01 ) had significant influence on unsatisfactory late result ( 30 cases ). Conclusions. In majority of displaced ankle fractures minimal osteosynthesis can be performed. The most decisive factors influencing the late result of operative treatment were the type of fracture, the accuracy of the reduction, the energetics of the trauma and the condition of soft tissues. Anatomical reduction and stabilization of each, even small, intraarticular fragment of third malleolus is indicated.
