Abstrakt     The computer system Viprecox (Vis et pressus in coxa, Forces and pressure in the hip joint) was developed for estimation of some geometrical parameters of the human pelvis and femur and calculation of the stress distribution in the hip joint articular surface. For this purpose the object-oriented programming language Java (Sun Microsystems, Inc.) was used. Basic data about the patients as well as digitized profiles of standard antero-posterior radiographs are used as input to the computer system. An alternative procedure was developed for automatical determination of digitized profiles directly from antero-posterior radiographs (the computer system Active Contours). The system Viprecox automatically estimates some important points, distances and angles that comprise the geometry of the pelvis and the femur. By using these parameters the system calculates and graphically represents the hip joint reaction force and the stress distribution in the hip joint articular surface in the frontal plane of the body. The calculation of the forces and the stress distribution acting on the hip joint is based on existing three-dimensional mathematical model of the hip joint reaction force and three-dimensional model of stress distribution in the hip joint articular surface. Additionally, the system allows visualization of the changes in the stress distribution when appropriate changes in the geometry of the hip joint occur, with respect to the implemented models. The functioning of the computer system was checked by calculating some geometrical parameters of the pelvis and the femur and the maximal stress in the hip joint articular surface (pmax) for 81 patients (37 male and 44 female) that were operatively treated for acute medial fracture of the femoral neck by implantation of hip endoprosthesis.
