Abstrakt     The aim of this study was to evaluate and compare sensitivity and specifity of CR and DDS in the detection of hemodynamically significant renal artery stenosis (RAS) >= 60% and to asses their positive predictive values for cure or improvement of hypertension by percutaneus transluminal renal angioplasty (PTRA). In the study 28 patients, aged from 32-68 years (average 50 years) with moderate or high clinical criteria for renovascular hypertension (RH) were included. In all patients CR, DDS and renal angiography were performed. Patients with angiographically proved RAS >= 60% were treated with PTRA. Results of CR and DDS were compared with renal angiography in 45 renal arteries. The prevalence of renovascular disease was 85,9%. Criteria for hemodynamically significant RAS in DDS were maximal sistolic blood flow in renal artery above 180cm/s and renal artery aorta ratio (RAR) > 3,5. Diagnostic sensitivity and specificity of DDS in detecting RAS >= 60% were 83% and 81%, positive and negative predictive values were 75% and 88% respectively. The accuracy of DDS was 82%. The percentage of technically failed DDS was 19,6%. In CR, criteria for hemodynamically significant RAS were lowering the relative renal function for >= 5% and/or prolongation of parenhimal transit time above 300 sec in comparison with renography performed in basal conditions, withouth Captopril. We found diagnostic sensitivity and specificity of CR in detecting RAS >= 60%, 78% and 81% respectivelly. Positive and negative predictive values were 74% and 85% and the accuracy was 80%. Positive predictive values of DDS and CR for cure or improvement of hypertension after treatment with PTRA were 85% and 86% respectivelly. CR and DDS are comparable tests for the detection of hemodinamically significant RAS in selected group of patients with moderate or high clinical suspiction for RH.
