Abstrakt     Leptospirosis occurs through the world and has multiple clinical presentations. European and USA cases seem partially different from East (Philippine, Thailand) ones. The aim of the study was to review clinical manifestations and outcome of Albanian patients (pts) with leptospirosis. Eleven pts with leptospirosis, 8 of them complicated with ARF, admitted to University Hospital Center of Tirana during 1993-1994, were studied retrospectively. All pts were male; their age distribution was 35.2 (14.8 years old. All pts had fever, weakness, jaundice, while vomiting, skin hemorrhage and conjunctival jaundice were present respectively in 6,6 and 8 pts. Eight of them presented with serum and urine abnormalities, suggesting renal involvement. Serum creatinine in those patients ranged between 3.8-9.3 mg/dl, while azotemia 180-340 mg/dl. Nine pts had hypotensive phase and seven pts had oliguric phase. The duration of diuretic phase was 5.7 +- 2.4 days and maximum urine volume was 4200 +- 1300 ml/day. Hemodialysis was performed in one patient, while peritoneal dialysis in two pts, during the period of azotemia. Weil's syndrome was observed in one patient, who died during its oliguric phase. The cause of death was shock. Ten pts had a complete recovery, four asked a period of 4-5 months. The clinical features and the frequency of renal failure during leptospirosis in Albania are similar to that reported at other European AND USA centers. The motrality in these patients resulted lower in our study.
