Abstrakt     The purpose of the study was to correlate the clinical course of laryngeal papillomatosis with the histopathologic findings of the removed papillomas. The authors investigated the role of infection with HPVl6/18 in the pathogenesis of risky atypical epithelial hyperplasia or malignant transformation of the papillomas. Among 123 patients; abnormal and/or atypical epithelial hyperplasia was found significantly more often in the papillomas of the patients with recurrent disease than in the patients in whom only one surgical procedure was necessary for cure: Laryngeal carcinoma appeared 9 times more often in adult patients with recurrent papillomatosis and risky epithelial hyperplasia than in the patients with recurrent disease without risky epithelium. Carcinomatous transformation of the laryngeal papillomatosis was detected always in the patients with adult type of disease. The correlation between HPV 16/18 infection and development of risky epithelium or carcinomatous transformation of the papillornas was not proven.
