Abstrakt     The anatomy of the floor of the mouth lympatics, especially in the relation to the periosteum of the mandible has still not been adequately defined, nor the possibility of lymphatic spread of the floor of the mouth cancer into the mandibular periosteum. In recent decades the opinion has begun to dominate, as regards planning operations for floor of the mouth cancer, that it is safe enough simply to exicise the tumour as local directly involve the periosteum. But even years ago we sometimes noticed that in the patient whose floor of the mouth cancer was completely removed with the help of checking with a frozen section, the disease recurred. This made it essential that we seek the cause for our lack of success. We concluded that the reason for the recurrence was probably lymphatic dissemination of the cancer through the lymphatic vessels into the mandibular periosteum and bone. The hypothesis was tested by means of retrospective and prospective studies. The retrospective part of the research shows the 3-year survival rate of 61 patients operated on from the beginning of 1977 to the end of 1986 at the University clinic for otorinolaringology and cervicofacial surgery in Ljubljana. Comparison of the 3-year survival rate of patients with floor of the mouth cancer in terms of spread of the disease and form of treatment showed that survival after segmental intervention or hemimandibulectomy and equal neck dissections, and radiation therapy despite the late stage of the disease (III and IV) was 50 II), where only excision of the tumour was performed as local surgical treatment. The prospective part od research comprised 31 postoperative dissection speciemns from patients with floor of the mouth cancer. Lightmicroscopic and immunohistochemical methods were used to investigate the lymphatic vessels. The link between the lymphatic vessels of the floor the mouth and the mandibular periosteum was proved by means of serial sections.(trunc.)
