Abstrakt     The teoretical part presents the progress in our understanding of the terminal enf of the common bile duct - the sphincter of Oddi (SO) - and its motor activity which is assessed by endoscopic manometry, cholescintigraphy and protracted transcystic manometry. The influence od pancreatic enzyme regurgitation on the bile ducts, particularly the influence of trypsine and its levels in the srum of healthy persons and patients with different diseases, are dealt with separately. In the review of SO function disorders, dysfunction of So motor activity is analysed in patients with cholecystolithiasis and choledocholithiasis (H-HDL). Determination of trypsine levels in the bile ducts of controls (KS) and investigated (HDL) group was aimed to confirm the author's hypothesis, and to establish the primary and secondary causes of SO function disorders in patients with H-HDL. The chapter on patients and methods explains the conditions qualifying the patients for KS of HDL group, characteristics of patients in both groups, the method of bile sample taking, and describes the method for trypsine determination and statistical analysis performed. In the chapter on results, the difference between clinical indicators and pathoanatomic features of the bile ducts in KS and HDL groups of patients, and the incidence of microlithiasis are presented; further, the possibility od stone passage through the cystic duct is assessed, and trypsine levels in the gallbladder and bile ducts in KS patients are stated. Trypsine levels in the gallbladder and bile ducts in HDL group is discussed separately for non -icteric patients with "floating" stones in the bile ducts, and separately for icteric patients. In the discussion, clinical indicators and pathoanatomic features in KS and HDL groups are correlated with the relevant findings of other authors, and the characteristics of patients under study are analysed.(trunc.)
