Abstrakt		The pharmaceutical profession will in the future be judged on the services it will be able to offer to detect potential harmful medication to the hospitalized and also the outpatient with the objective to prevent hospital-admission or re-admission. We are all subjected to the rapid changes in the health environment and are confronted with the closing of hospitals, with the dramatic reducing of hospitalbeds and the annexed services. On the other side, home care and home hospitalization are developing rapidly. It is most important for clinical pharmacy to follow these movements and to bring the pharmaceutical quality assurance developed in the hospital-surroun dings also to the patient at his home and o those living in nursing homes.If we are not able to innovate the teaching in our faculties, our profession will face an increasing risk of sclerosis. To prevent this happening, we need to bring pharmacy students more and more in contact with pharmacy practitioners and this at University level. Only when the pharmacist gets strongly involved in clinical/ pharmaceutical care activities will he acquire the status of a real specialist complimentary to the existing medical disciplines.
Izvleček		Farmacevtski poklic bo sodobna družba vrednotila predvsem po storitvah, ki jih bo nudil pri odkrivanju potencialno škodljivih učinkov zdravil v populacijah hospitaliziranih, pa tudi ambulantnih pacientov. Cilj teh dejavnosti bo med drugim zmanjšanje verjetnosti hospitalizacije ali ponovne hospitalizacije. Vsi smo soočeni s hitrimi spremembami v zdravstvenih sistemih, med drugim tudi z zapiranjem bolnišnic, dramatičnim zmanjševanjem števila bolniških postelj in dodanih storitev. Na drugi strani se povečuje pomen zdravljenja na domu, pa tudi hospitalizacije na domu. Za klinično farmacijo je zelo pomembno, da sledi tem spremembam in da prenese sisteme zagotavljanja kakovosti ki jih je v preteklem obdobju razvila v bolnišničnih okoljih, tudi pacientom, ki se zdravijo na domu in tistim, ki živijo v domovih za ostarele. Če za te cilje ne bomo uspeli prenoviti vsebine študija farmacije se bo naš poklic soočal z naraščujočim tveganjem sklerotičnosti. Da bi to preprečili moramo vzpostaviti boljši stik med študenti farmacije in lekarniškim farmacevti v praksi in to na univerzitetni ravni. Šele ko bo farmacevt tesno vključen v aktivnosti klinične farmacije in farmacevtske skrbi, bo pridobil status specialista za področje zdravil, komplementamega obstoječim medicinskim diciplinam.
