Abstrakt     Purpose. Complete knee dislocation is a limb threatening injury with high proportion of popliteal artery and tibial/peroneal nerve damage. Conservative treatment is possible, but often leads to knee instability. The purpose of the study was to evaluate conservative and operative treatment for different types of dislocations. Methods. Patients treated for knee dislocation from 1993-2000 in our hospital, were evaluated. They were devided into groups according to predominant medial or lateral instability. At final examination they were evaluated for mediai or lateral and anteroposterior instability, range of motion and function. Results. 7 patients had popliteal artery occlusion. In two cases, amputation at the level of the knee was performed. In 5 cases, the vessel was succesfully repaired. In 5 cases tibial or peroneal nerve was damaged. Of the 15 patients with salvaged limb there were 9 patients with predominant medial/AP instability and 6 patients with lateral/anteroposterior instability. Conservative treatmennt in 3 cases of medial/AP instability gave good results, but unstable knees in both eases of lateral/AP instabilities. Immediate operative reconstruction of all ligamentous structures gave good results in all cases. Conclusion. Knee dislocation is a limb threatenlng injury. Vascular damage should be suspected in all cases of knee injuries with multidirectional instability. Vascular repair can be succesfull, but should always be followed by fasciotomy of all muscular compartements. Dislocated knees with predominanat lateral/AP instabilities should be operatively treated. Good results can be expected if all structures are acutely repaired.
