Abstrakt     Background. Less than 10% of births occur preterm but they are the major cause of postnatal death and handicap. Prematurity is implicated as cause of death in more than 75% of all neonatal deaths. Different scoring systems for screening of preterm delivery were developed in the past mainly based on demographic, social, medical history and on clinical signs and symptoms in present pregnancy, but most are ineffective because of low sensitivity (40%) at a too large screen positive group (15%). Home uterine activity monitoring has aslo been abandoned as a screening tool for preterm labour, because most studies demonstarted significant increase in uterine activity only 24-48 hours before onset of irreversible preterm delivery. Methods: Under the auspices of Fetal Medicine Foundation at Kings College in London, vaginal ultrasound has been introduced systematically as a screening tool for preterm delivery. In a series of pilot studies they could demonstrate at least 50% sensitivity for serious preterm delivery with a cervical length of <-15 mm at 23 weeks of pregnancy and after they have put a stitch on the cervix in a small group of women with a short cervix they noticed a dramatic drop of preterm delivery in comparison to the group without a stitch. They designed a multicentre, randomized and controlled intervention study to research the role of the cervical cerclage in preventing preterm delivery before 32 weeks of pregnancy. In the study Ljubljana perinatal unit is a participating centre. Results. Our partial results on 1622 screened pregnancies show that the gestation achieved in women with a short cervix was statistically significantly shorter than in those with a normal length of the cervix. Our results on 14 randomized women with a short cervix also demonstrate a significantly shorter gestation in those without a stitch compared to those who received a stitch.
