Abstrakt     Blood cholesterol lowering with statins has been regarded as a long term strategy to reduce death and ischemic cardiovascular events in patients with stable coronary artery disease. In conjunction with lowering total and LDL cholesterol, statins may improve endothelial function, decrease platelet aggregability and thrombus deposition and reduce vascular inflammation. Each of this effects might be expected to have favorable impact in the acute phase of the coronary event. This hypothesis was recently addresed by the *MIRACL* trial conducted in 3086 patients with unstable anginalnon ST elevation myocardial infarction who received either treatment with atorvastatin (80 mg/day) or matching placebo within 24 to 96 hours following hospital admission. There were no significant differences in risk of death, nonfatal myocardial infarction or resuscitated cardiac arrest between the atorvastatin group and the placebo group. Atorvastatin group had lower risk of recurrent symptomatic ischemia with objective evidence and requiring emergency rehospitalization. There was no significant differences in the incidence of secondary outcomes of coronary revascularization, worsening heart failure or worsening angina. There were, however, fewer strokes in the atorvastatin group than in placebo group. No serious adverse event occurred with a frequency of more than 1 % in either group. Accordingly, early initiation of statin therapy is feasible and may reduce recurrent ischemia without significant effect on hard endpoints such as death, nonfatal myocardial infarction and resuscitated cardiac arrest.
