Abstrakt     Introduction. Nowadays, the diagnosis of threatening preterm delivery is predominantly based on clinical evaluation, which is often inaccurate or belated. Investigation of electrical uterine activity, which can be detected by electromyography (EMG), provides a promising diagnostic method for early detection of threatening preterm delivery. The aim of this study was to find and assess whether the EMG signal, acquired in two directions (longitudinal and transverse), provides useful information on the activity of the uterine corpus, which leads to threatening preterm delivery. Besides, we wished to find whether there is a difference between the contraction of the uterus in longitudinal and circular directions, leading to preterm delivery and that not resulting in preterm delivery. Methods. The study involved 29 parturients admitted to the Department of Obstetrics and Gynecology in Ljubljana for threatening preterm delivery. Within a few consecutive days we performed 53 EMG measurements. With a measurement system, created for the purpose, the data of EMG signal was acquired by four bipolar electrodes positioned on the surface of the abdominal wall: upper transverse pair (first channel), longitudinal pair (second channel) and lower transverse pair (third channel). With a special computer software for acquisition and preparation of the acquired signals, characteristic parameters of the EMG signal were calculated, viz. the Root Mean Square voltage (RMS) and median frequency (MF). Results. The analysis of EMG parameters showed that in preterm delivery longitudinal RMS is greater than transverse RMS. in term delivery this difference is almost not noticeable. Logistic model, built on the study group using the parameters of EMG uterine activity, showed that the difference between the uterine activity in the longitudinal and the one in the transverse direction improves the predictive value for preterm delivery.
