Abstrakt     Background: At present, the adjuvant treatment of breast cancer is tailored to the patients characteristics and to those of tumors. This treatment modality is therefore based upon the prognostic factors that predict the course of the disease as well as upon the predictive factors that predict patients response to specific systemic treatment. The patients with unfavorable prognosis, which is usually based upon a high number of positive axillary lymph nodes, benefit the most from adjuvant systemic treatment. However not all the patients with a high number of positive axillary nodes have an unfavorable prognosis; therefore, for an optimal treatment planing of this group of patients, a more specific analysis of their prognostic and predictive factors is needed. Purpose: The primary aim of our retrospective study was to find out whether a group of patients with more than three positive axillary lymph nodes can be further devided into more specific prognostic subgroups. Therefore, in addition to the prognostic impact of the classical prognostic factors, we also analyzed the prognostic impact of four potential prognostic factors (DNA ploidy of the tumor, S-phase fraction, the fraction of MIB-1 positive cells and HER-2 receptor.) Further on we wanted to find out whether any of the potential prognostic factors under study could be also used as a predictive factor under study could be also used as a predictive factor of the response of patients with unfavorable prognosis to adjuvant systemic treatment with antiestrogen tamoxifen, to chemotherapy according to CMF scheme and to the combination of these two treatment modalities. Patients and Methods: One hundred and forty-six patients with operable breast cancer were included into the study. All the patients were primarily treated at the Institute of Oncology in Ljubljana in the period from 1989 to 1993. (Abstract trunacted at 2000 characters).
