Abstrakt | This review article is intended to introduce the uninitiated pediatrition to the basic concepts, aims and questions of the clinical genetics of hypertension. It separates the rare monogenetic hypertension disorders from the vast majority of patients with essentional hypertension, which is a complex, polygenic, multifactorial disorder resulting from interaction of several genes with each other and with enviroment it highlightssome familial studies, the role of ethnicity, sodium, sympathetic system and histocompatibility antigenes. Genes implicated in the etiopathogenesis of essential hypertension (NHE-1 gene, renin gene, ACE gene, AT gene, S gene, NOS gene, atrial nutriuretc peptic gene, alpha-adducin gene, insulin receptor gene and angiotensinogen gene) are briefly dicsussed. The five different forms of monogenetic hypertension (Liddles syndrome, Gordons syndrome, glucocorticoid-remediable aldosteronism, the syndrome of apparent mineralocorticoid exess and the syndrome of brachydactyly type E and hypertension) are liszed. It is hopped that new, more efficient methods of genetic analysis will Yield clinically meaningful information. | |
