Abstrakt     We report on a case of a desmoplastic small round cell tumor (DSRCT) of paratesticular region in a 57-yr-old patient presented to us in consultation. The tumor of 12 cm in its greatest dimension was intimately associated with tunica vaginalis testis and caused a huge hydrocele. Retroperitoneal and mediastinal lymph node metastases were seen on CT. An orchidectomy was performed in another institution. Microscopically, it showed typical morphologic features of DSRCT and immunohistochemically exhibited characteristic divergent immunophenotype including positivity for some epithelial markers, desmin, and neuron-specific enolase. The patient showed complete response to chemotherapy on CT and is presently without evidence of disease. Previously, 13 cases of paratesticular DSRCT were reported, and in some of them excellent response to chemotherapy was observed. It was suggested that DSRCT of paratesticular region might have better prognosis than those in classical intraabdominal location.
