Abstrakt     Background. Confocal microscopy is a diagnostic method used for exannination of the morphological structure of the cornea in vitro as well as in vivo. Changes less than 1 micro m of site can be observed, at a level of cell. Using a confocal microscope, we can determine and analyze morphological changes in individual layers of the cornea, from epithelial to stromal and endothelial ones; it is possible to observe keratitis causing agents. Based on the findings, we choose the method of treatment and the type of surgical procedure. The method is clinically applicable also in an opaque cornea occurring as a result of epithelial or stromal edema. Such investigations were not feasible before the introduction of confocal microscopy. The investigation is aimed to determine cellular density in different layers of the cornea, i.e., that of epithelial cells, stromal keratocytes and endothelial cells. Monitoring cellular density by layers helps us understand the cell response after corneal damage or surgery of the anterior segment of the eye. Methods. Our study was aimed to compare keratocyte density and the area of keratocyte nuclei in the central segment of the anterior, medial and posterior layers of corneal stroma. The study population consisted of patients after corneal grafting (20 eyes) and a control group of healthy persons (24 eyes). Comparison by age groups was done on the basis of donors age as registered in surgical protocols at the time of corneal graft taking. Operations were performed by the same surgeon. We used a Scanning Slit confocal microscope (ConfoScan 2.0). The objective consists of a lens with magnification of 40x and numerical aperture of 0.75, the two constant values determining the depth resolution of the optical section examined, which is 10 micro m.
