Abstrakt     Osteosarcoma, a rare malignant tumor, has been, less than thirty years ago, considered as resistant to treatment. More than 80 % of patients died due to metastases, despite radical surgical treatment. With the introduction of chemotherapy survival has significantly improved and so has the quality of life with more conservative surgical techniques. But still, almost one half of the patients with osteosarcoma die of disease. It is important to find prognostic factors that influence survival and consequently to adjust therapy accordingly. Patients with better prognosis should be treated more conservatively, without jeopardizing their survival, and for patients, in whom the present therapy is unsuccessful, new approaches have to be found. In our retrospective study, the influence of prognostic factors on survival of patients with osteosarcoma was assessed. Based on data from Cancer Registry of Slovenia and Register of the Institute of Oncology in Ljubljana, 74 patients were included in the study. They were S -18 years old at diagnosis and treated between 1968 and 1997 at the Institute of Oncology (OI) and/or University Children Hospital- Medical Center (PeK) in Ljubljana. Based on our experiences and on published data, we analyzed gender, age and stage at the diagnosis, primary site, histological tumor subtype, level of alkaline phosphatase and lactate dehydrogenase in blood as possible prognostic factors, influencing survival. The influence of tumor necrosis after preoperative chemotherapy, kind of treatment and recurrence of disease on survival have also been analysed. The overall survival of patients with osteosarcoma is 50 % and the results are comparable to results, published worldwide. The stage of the disease and treatment including chemoterapy are the only prognostic factors that significally influenced the survival in our series.
