Abstrakt     Lyme borreliosis is the most common human tick-borne infection in Slovenia. It is caused by B. burgdorferi sensu lato. The number of registered patients is increasing every year. Erythema migrans is a skin rash characteristic of early Lyme disease, which enables an easy and accurate clinical diagnosis. Isolation of B. burgdorferi sensu lato from the skin lesions is still the most accurate method of the confirmation of the clinical diagnosis of the erythema migrans. In Slovenia 4 species are known to be associated with the Lyme borreliosis: B. burgdorferi sensu lato, B. afzelii, B. garinii and B. bissettii. According to the data published in the literature different genospecies of B. burgdorferi sensu lato are associated with different manifestation of Lyme disease. In the recent study we wanted to confirm our hypothesis that the course of erythema migrans depends on the species of B. burgdorferi sensu lato. In our geographic region almost 80% of erythema migrans is caused by B. afzelii, followed by B. garinii in 9 to 17% while the other two genospecies are not frequently present. European authors found out that B. garinii is more invasive and more often affects the central nervous system than other borrelial genospecies pathogen to humans. Because of the small number of both patients with B. burgdorferi sensu stricto and B. bissettii isolated from erythema migrans, we were only able to compare the course of the illness in patients with erythema migrans caused by B. afzelii and B. garinii. 200 adult patients with erythema migrans and isolation of B. afzelii from the skin lesion registered in 1997 at the Department of Infectious Diseases in Ljubljana and 53 adult patients with B. garinii positive isolation from erythema migrans treated at our department from 1993 to 2000 were included in the prospective clinical study.
