Abstrakt     1.) Introduction: Schizophrenia is difficult mental disorder, it affects just under 1 percent of the world's population. It is a clinical syndrome of variable psychopathoiogy, which involves thought, perception, emotion, volition, movement, cognitive deficite and negative symptoms. Diagnosis is met on grounds of typical signs, symptoms and course of the disease, which is difficult because of the heterogeneity of clinical presentation. Routinely used diagnostic tools, psychiatric interview and psychological te*ting are often not enough for making the diagnosis. Etiology of schizophrenia is unknown. Altered regional cerebral blood flow (r-CBF) is mentioned as one of the possible etiological factors. Perfusion scintigraphic brain studies with SPECT (Single Photon Emission Computed Tomography) help us to recognize areas of the brain with altered perfusion during the disease. 2.) Aim: Aim of the study was to confirm the hypothesis that brain perfusion of acutely ill schizophrenic patients in the early stage of the disease is altered and that it differs from perfusion in later stages. Therefore, we evaluated regional cerebral blood flow (r-CBF) in acute first-episode schizophrenia and the effect of antipsychotic drugs on r-CBF. We examined the correlation between psychopathological symptoms and r-CBF. 3.) Methods: Clinical criteria for schizophrenia were met according to International Classification of Diseases - 10th Edition (ICD-10). All patients were male. Psychic status and severity of the disease in each patient were evaluated with a semistructured psychiatric interview, Positive and Negative Syndrome Scale (PANSS), Clinical Global Impression (CGI), and unwanted effects of antipsychotic drugs were evaluated with ESRS (Extrapyramidal Symptom Rating Scale) on the same day as the scintigraphic study.
