Abstrakt     Assessment of small-fibre function was made with thermotest (Marstock method) in healthy volunteers, diabetic patients without symptromatic neuropathy and diabetic patients with symptomatic neuropathy. Healthy volunteers and diabetics and both groups of diabetic patients differed in sensitivity for cold, warm, cold pain and heat pain. Disorders of small fibre function were found in some elderly healthy volunteers as well, which might be a consequence of subclinical age-related neuropathy. Walking ability was assessed with a 6 - minute walking test. Results differed significantly among the three groups: healthy volunteers performed best, and diabetic patients with symptomatic neuropathy worst (p < 0.001). We discovered that walking ability is influenced by sensory disorders. The distance covered in 6 minutes is significantly shorter in the presence of sensory disorders on the dorsal aspect of the foot (p < 0.05). Influence of sensory disorders of the thenar, thigh or leg on walking ability was not proved (p > 0.05). Foot Function Index was used for the assessment of pain, disability and activity limitation. The questionnaire is suitable for the assessment of pain and disability in diabetic patients. Foot pain correlates significantly with the result of walking test (r = -0.449, i = -0.333, p < 0.001). Patients with worse foot pain have more difficulties when walking long distances than patients with milder pain or those without pain. Only 10 - 20% variability of the walking test score can be attributed to foot pain.
