Abstrakt     A comparative study of two surgical techniques was undertaken to demonstrate that operative repair utilizing a patellar ligament transplant with preservation of the torn ligament as a support for the transplant is superior to the repair by suturing the ruptured ligament. The results of the latter technique are insatisfactory as shown by the high incidence of the Lachman sign exceering 4mm, anterolateral instability and restricted range of motion. Early diagnosis of knee injuries is mandatory. Patients with rupture of the anterior cruciate ligament should be treated by surgical repair using a patellar ligament transplant. At the Department of traumatology, University medical centre Ljubljana, 186 patients with the diagnosis of fresh rupture of the anterior cruciate ligament, aged 20 to 60 years, were followed in the years 1984-1987. They were all summed for a control examination 12 months after the operation, but only 182 attended. The patients were evaluated by physical examination, diagnostic tests of the affected knee joint, measurements of the thigh muscles and the Lachman test. There were 115 men (63,2 percent) and 67 women (36,8 percent). In 31 patients, the knee was treated by suturing of the torn ligament, while in 151 patients operative repair utilizing a patellar ligament transplantat was done. There was a statistically significant difference between the groups as concerns the man values of the Lachman test (p less than 0,0005). A pivot shift phenomenon was compatible with the Lachman sign. Since the pivot shift sign was positive with values greater than 4mm, it was replaced by the Lachman test. The most common mechanism of trauma in men was internal rotation and varus, while in women the injury was mostly due to external rotation and valgus. The differences between males and females were statistically significant (p less than 0,0005).(trunc.)
