Abstrakt     The effect of the Chiari osteotomy on the distribution of the contact hip stress over the weight bearing area is studied. A mathematical model is constructed where additional coverage of the femoral head by ala ossis ilei is taken into account. It is shown, that this additional coverage importantly changes stress distribution by changing the position of the stress pole. Therefore the weight bearing area increases due to two reasons: directly due to the additional area formed by the ala ossis ilei segment and indirectly (on the medial side) due to the change of the position of the stress pole. The indirect effect is important and often larger than the direct one. Using standard anteroposterior radiographs from the archives and the proposed mathematical model, the hip stress distribution was determined before and after the Chiari osteotomy on a population of 30 dysplastic hips and compared to the results for the population of 30 healthy hips. Peak stress on the weight bearing area in the population of dysplastic hips is considerably higher than in the population of healthy hips (for aproximately 2,3 times). After the Chiari osteotomy the peak stress considerably (for about a half of the initial value) and statistically significantly (p<0.001) decreases indicating favourable biomechanical effect of this operation.
