Abstrakt     For thrombolytic treatment of arterial occlusion several thrombolytic agents and different therapeutic schemes are used. However, not even local application of thrombolytic agents, that ensures high concentration of the agent at the thrombus site, can assure 100 percent therapeutic success. This indicates that the success of treatment depends not only on thrombolytic agent used but also on other factors. In order to improve success of local thrombolytic treatment it is important to identify factors which determine therapeutic outcome, specially those factors that are important for therapeutic failure. Therefore, we investigated factors and conditions that, besides thrombolytic agent, influence early and late results of local thrombolytic treatment. We expected that knowledge of these factors would enable more adequate selection of patients for this type of treatment and to remove unfavourable influences that are responsible for reocclusions. We studied: - influences of haemodynamic conditions (localization and lenght of occlusion, state of arteries distal of occlusion - outflow), influences of etiology and age of vessel occlusion, influence of diabetes and other risk factors for atherosclerosis for success of local thrombolytic treatment of arterial occlusion (I. study); - if enrichment with exogenous plasminogen improves therapeutic success (II. study); - if simultaneous application of heparin and streptokinase prevents frequent ascending thrombosis along intraarterial catheter which decrease recanalisation rate (III. study); - which factors influence reoclusion rate after successfull recanalisation of arterial occlusions (IV. study). In all studies great majority of patients had heavy circulatory disturbances of limbs with critical ischaemia. Included were only those patients who were refused by vascular surgeon because of unsuitable vessels for reconstructive surgery.(trunc.)
