Abstrakt     INTRODUCTION. In order to diagnose rheumatoid arthritis (RA) and to determine the success of treatment, one uses roentgenograms of the peripheral joints, most often the posterioanterior roentgenograms of hands since the changes on these joints are most reliable indexes of RA's progress. The development of bone erosion, cartilage damage and dislocation can be illustrated by repeating the radiographic research. To get a more accurate assessment of the extent of joints' damage, and to assess the development of the illness more accurately, one uses a standardised scoring system, which gives reliable results. However, none of the scoring systems has been generally accepted so far. METHODS, MATERIALS, PATIENTS. In our retrospective research we compared the Sharp's scoring system to the Scott's scoring system and to the carpo-metacarpal (CMC) ratio system. The Sharp's scoring system grades the extent of erosion and joint space narowing (JSN), while the Scott's scoring system grades the amount of dislocation (subluxation and luxation) of joints as well. The CMC-index is the ratio between the wrist length and the length of the third metacarpal bone. The ratio decreases when destruction of the wrist occurs. Our research was conducted on roentgenograms of 20 patients, whom we treated at the Rheumatology Clinic of the Peter Držaj Hospital and for whom final clinical, laboratory and roentgen diagnosis of erosive RA were made in the period between 1st May 1999 and 30th April 2000. When diagnosed, the patients' ages varied from 32 to 77 years (58,5 years in the average), and the illness had lasted from 1 to 36 years (12,8 years was the average duration). Intraobserval testing was performed, and roentgenograms were scored three times one month apart according to the three different methods. Correlations, regression analysis and range were used for processing the statistical data. Statistical significance was tested by using the t-test. (Abstract truncated at 2000 characters).
