Abstrakt     A prospective study of decentralized spinal cord sympathetic reactivity was pertormed in 13 patients with complete spinal cord injury (ASIA group A) at the spinal segment Th4 or below. Sympathetic skin response (SSR) of the feet after electrical stimulation of mixed nerves, femoral and tibial, and of low extremity sensory nerves was used as an indicator of sympathetic spinal cord reactivity. Afferent excitation from S2-3 peripheral segments was tested by dorsal penile nerve electrica! stimulation. In 7 out of 13 patients we confirmed preserved sympathetic spinal cord reactivity after strong electrical stimuli (lms 300V), and found more frequent positive feet responses after mixed nerve stimulation compared to smaller, pure sensory nerves. An exception is dorsal penile nerve which, when stimulated, evoked feet SSR in all 7 responsive subjects. The method is recommended as additional to routine neurophysiological investigation of paraplegic patients. It selects sympathetically reactive patients who would benefit more than other SCI patients from routine physiotherapy: tactile stimulation during kinesitherapy and functional electrical stimulation.
