Abstrakt     Background: Complex talar injuries may result in partial or complete avascula necrosis of the talus. In open injuris and after operative procedures septic complications are common. Most complex talar injuris later develop arthrosis. Methods and results: Six recent interesting cases of complex talar injury, their treatment modality and the late results are presented. Conclusions: In cases where bone vitality and morphology is preserved and there is also a sufficient soft tissue coverage, artificial ankle is usualy a succesfull solution, but a sound ankle arthrodesis and an eradication of bone infection is a fair result for the others. Sometimes even the second option presents a challenge. Concomitantly one must consider the indication for a simultaneous subtalar or pantalar fusion.
