Abstrakt		Purpose. To evaluate the clinical outcome of anterior cruciate ligament reconstruction using the technique termed "posteriorly limited notchplasty". Type of studies. Case series. Methods. This follow-up study included 55 patients undergoing anterior cruciate ligament reconstruction with the posteriorly limited notch plasty using the patellar tendon graft with its bony attachments. Clinical results were evaluated using the IKDC and Lysholm rating scales, and KT-1000 arthrometric testing of ligamentous laxity. The patients attended for the final follow-up testing on average 30 months after the reconstruction (range; 13-52 months). Results: The mean final Lysholm score was 94.6, and the median final Lysholm score 95. According to the IKDC scoring form, 18 patients (33%) were classified as grade A, 33 (60%) as grade B and 4 (7%) as grade C. The pre-injury activity level was resumed by 32 patients (58%); in 20 patients (36%) the level of activity after the reconstruction decreased by one grade and in 3 (6%) by two grades. At the final follow-up examination, the mean laxity grade of the reconstructed knee determined by a KT-1000 arthrometer was by 1.3 mm greater than that of the contralateral joint. Conclusions. Clinical results of the anterior cruciate ligament reconstruction based on biomechanical studies are encouraging.
Izvleček		Namen dela. Ugotoviti klinične rezultate rekonstrukcije sprednje križne vezi z zadaj omejeno notch plastiko. Tip raziskave. Serija primerov. Metode. Spremljali smo 55 poškodovancev po rekonstrukciji sprednje križne veži z delom kit pogačice s pripadajočima kostnima narastiščema in z zadaj omejeno notch plastiko. Klinični rezultati so bili ovrednoteni z uporabo točkovnih sistemov IKDC, po Lysholmu in meritvami ohlapnosti z artometrom KT-1000. Povprečen čas spremljanja je znašal 30 mesecev po opravljeni rekonstrukcij (rang 13-52 mesecev). Rezultati. Povprečna končna ocena po Lysholmu je znašala 94,6 točk, mediana pa 95 točk. Po ocenitvenem sistemu IKDC smo 18 poškodovancev (33%) ocenili z oceno A, 33 (60%) z oceno B in 4 poškodovance (7%) z oceno C. Nihče ni dobil ocene D. Na isto aktivnostno raven (merila po IKDC kot pred poškodbo se je po rekonstrukciji vrnilo 32 poškodovancev (58%, 20 (36%) je zmanjšalo aktivnost za eno, 3 (6%) pa za dve stopnji. Povprečna vrednost ohlapnosti operiranega kolenskega sklepa, izmerjena z artrometrom KT-1000, je znašala na zaključnem pregledu 1,3 mm več kot na nasprotnem kolenskem sklepu. Zaključek. Klinični rezultati rekonstrukcije, pri kateri kirurška tehnika sledi rezultatom biomehanične študije, so spodbudni.
